refactor: B011 修复 - assert False 替换为 raise AssertionError

- 6 处 assert False, "msg" 改为 raise AssertionError("msg")
- 避免 python -O 模式下断言被移除导致测试逻辑失效
- 符合 ruff B011 规则要求
